Orthodontic Referrals
Background
Our practice has been established for over 20 years and receives more than 2000 referrals a year from over 100 dentists.
We do have a growing waiting list for new NHS patients - see our waiting list policy - but no treatment or private patient waiting lists.
To be eligible for NHS treatment, patients must score 3.6 or above on the Index Of Treatment Need (IOTN).
We offer NHS treatment, in routine cases, up to 18 years of age but patients must be under 16 to be accepted onto the NHS new patient waiting list (because it's about 2 years long).
If a 16 or 17 year old patient needs an Orthodontic assessment, they should be advised that treatment will only be available privately.
We are pleased to enjoy a close working relationship with the local hospital based Consultant Service and a considerable level of cross referral exists between our two services.
These days, there is general agreement that as far as NHS provision is concerned, there should be a tripartite system of supply of Orthodontic treatment. The GDP's should carry out very simple courses of treatment, if they feel confident to do so. Specialist practices like ours, should carry out the great bulk of routine malocclusion and perhaps some of the simple work. The Consultant Service exists to treat severe and combined Ortho/surgical cases.
At Chapel Road, we are pleased to fit into the above pattern, which leaves all aspects of the service doing what they do best.

Non extraction, Arch development
Dr Horobin uses a tried and tested arch development technique, for mild to moderate class I and class II crowded cases, in the late mixed dentition.
The technique is one first developed by Cetlin and TenHove in the 1980's and recently popularised, in mainstream orthodontic circles, by Counihan.
An excellent article, by Counihan, can be found in the Journal of Clinical Orthodontics, June 2000. This shows the technique in great detail.
The essence of the technique is careful A/P and lateral expansion of the developing arches, over 2 or 3 years, with the aim of accommodating all the teeth into the arch as they erupt.
Lip bumpers, distal movement with headgear and "nudgers" and straight wire mechanics are all used in this complex but very reliable method.

When to refer
All child patients should be assessed orthodontically, by their own family dentist, at 9 years of age.
At this stage, a panoramic radiograph should be taken and any potential problems identified.
If a problem is found, early referral is preferable to late referral.
Nine years of age is the best time for orthodontic referral.
Referral should always be made prior to the loss of the final deciduous teeth, since many "boats can be missed" if all the deciduous teeth are all gone.
